As the title suggests, each page in Mrs. Muran's book consists of fill-in-the-blank entries for recording genealogical information on each member in the family. Information called for includes such things as name, address, date and place of birth, weight and height at birth and at present, medical information, nicknames, spousal information, children, previous marriages, special memories, schooling, careers, hobbies, pets, travels, special moments, personal philosophy, religion, and much more. The Questionnaire is divided into separate sections for the subject, his/her spouse, children, parents, siblings, aunts and uncles, grandparents, grandchildren, the extended family, and various kinds of personal information. At the back of the questionnaire is a special section for compiling miscellaneous information and for inserting and describing family photos.
